According to General Directorate of Health Office, beginning from 1st April 2010 Maritime Declaration of Health Form has to be filled by Masters or Owners and submited via fax or e-mail directly to the General Directorate of Health Office in the related Turkish ports.
Please note that beg of 1st April none of Turkish ports sanitary offices will accept Maritime Declaration of Health Form from vessels' agent.
